After you download and print the Adobe Acrobat file below, you will need to read, fill out, sign and send in all applicable forms. The file includes:

Here's how to get started:
1. Download the Application Form now >> ( You may need to download and install Adobe Reader )
2. Print and completely fill out the forms. The "Doctrinal/Risk Statements" are for you to read and then keep for your records.
3. Give a "Confidential Reference Form" to four individuals, including your pastor/church leader and have them return these to the address below.
4. Fax or e-mail all completed forms to the following:

...or you may also email a high resolution picture to missiontrips@ntm.org as well. Please mail in your Registration fee and a picture you want us to use for your colored, personalized prayer cards to:
